State employees or members of the public who wish to report incidents that fall under the purview of OLES are welcome to do so on this form. Upon receiving a complaint, OLES may use information that you provide to investigate the allegation(s) and may share your information with other government agencies as permitted by law to further the investigation of your complaint.
